Air Ambulance Setup in Ben. Admin

We are starting to setup Air Ambulance benefits in Ben. Admin. Automated Enrollment.  We have two Air Ambulance vendors (Life Flight and AirMedCare).

One of the vendors is requiring that we include multi-year enrollment options (1,3,5,10 years), an attestation statement, and they want us to provide dependent information since the coverage is for dependents as well.  The multi-year and attestation statement shouldn't be a problem, but I'm wondering which plan type would allow addition of dependents for a benefit that is not Health related (Plan Type 1x) or Life related (Plan Type 2x).  Or would you recommend I use one of those plan types?
