Exachk/Orachk provisioning failed in OEM 13.2

I am trying to integrate the exachk into OEM 13.2 with latest plugin deployed in OMS but it fails with following error. Any help to fix the issue would be highly appreciated if someone has any thoughts and have implemented successfully.
