Oracle Enterprise Manager 13c Migration

We have OMR(11.2 ) and  OMS(12.3 , no BI) on redhat 5 machine .

We now create new database for OMR(12.1 ) and  OMS(13.3) on redhat 7 machine .

Now we want to migrate to 13c (latest13.3)  from 12c(12.3),

Documentation explains only when upgrade happens on the same host where the earlier release of OMS exists.

What are the best possible approach to migrate from old machine to new machine .
