Req line to multiple PO

Hello.

We are on 9.1 and have an issue were it appears that Req line was sourced to PO (PO# 61784 below) and then was able to someone also get sourced to a new PO recently (PO# 62648).

Anyone have a good starting point of how an issue like this might happen?  I saw a few SRs related to the requisition change process - but we don't use that. We just sourced approved Reqs to PO.
