Unable to reserve document (Requisition) R12

Issue: Unable to reserve document (Requisition)

We are on R12.5+ and doing requisitions and hit a workflow error "Unable to reserve document"

We have Use Requisition Encumbrance set on the financial options for the organizations

We have budgetary control set to None

We funded the LOA in GL.

We tried override it did not work and tried reserving Again

Using employee->Supervisor approvals

Owner can approve on document type

any help is appreciated.
