9.2 TAM - Submitted Application with no Resume

We are on v9.2 TAM/CG and Tools 8.56.16

We encountered issue where applicants were able to successfully submit applications without resumes, even if applicants uploaded resumes. We figured "Data Integrity Error" was being ignored by applicants and continued thru' the application process. After upgrading Oracle DB to Unicode, the issue was fixed for a while.

However, we are encountering same issue where the answers to questionnaire (required at SITE level) are not being saved along with resume not getting attached, however, application was accepted by the system.

Unfortunately, we are unable to replicate the issue despite trying to submit different types of resume (format, file format, etc)?
