DBUA for 12c upgrade not showing source database details.

Hi Team

  I would like to upgrade from 11.2.0.4 to 12.1.0.2 . But unable to find the source database listed when I invoked dbua.

  I have infact attached the 12c home again in central inventory using following commands but there is no change.

  $ORACLE_HOME/oui/bin

  ./attachHome.sh

I have enclosed the document. please help.
