Add Shipped Item as Child to Parent Instance

Hello,

We have a requirement to Add Shipped Component as Child under Parent Instance from Sales Order.

I tried Installation Details Form to capture Parent Instance, but we need to provide replacement Relationship. I just selected an Item and shipped the Order.

Parent Child relationship is not created. Please help with this.
