Transfer Orders

Has anyone ever had an issue where two separate sales transfer were created BUT ended up on the same order transfer? The two sales transfers were entered on the same day and that is the only thing they really have in common.  They were entered about ten minutes apart.  We are trying to figure out how this could have happened.

Also side note:

They are different business units and companies.
