SQL Monitor installation to check sql query performance

Hi Gurus,

Can you help me with the documentation about installing SQL Monitor to check sql queries performance on 11g/12c  Database.

Does it come as part of Database installation software and installed on server or it is a separate download to install on client machines ?

Please provide me some Oracle Note IDs or documentation where I can find info on installing to monitor 11g/12c Databases.
