ECO is not implementing item revision correctly.

in Discrete Manufacturing - EBS (MOSC) Question

Item revision is done through ECO, but in some ECO item revision in not implemented in all orgs under org hierarchy. Some times its implemented in only one org or in 2 to 3 org only. Has somebody encountered such behavior of ECO. IT is a bug or some setup issue?
