ol 7 sudden crash, rejecting i/o to offline devices

I encountered i/o error as above on one of out SAS servers. The only solution is to hard reset, but then it will happen again after a few days.

There is no amber light or physical indication of hardware degradation but SAS support insists on checking it with hardware support.

Anyone can shed some light on this error? Where I should check next aside from /var/log/messages? There is no error in /var/log/messages.
