Financial reporting creating duplicate data source

Hi gurus.

Has anyone seen an issue like the following below?

In our PROD environment, we have an FR data source with the name - HFMPRD, but today it got changed from HFMPRD to HFMDEV2 and the connection name was changed from HFM to HFM (2). Not sure if there's a way to trace this back to what caused it?

Any thoughts?
