Sales Order issue

Hello Folks, Server details Application release - 9.0 with TR 9.1.5.5 on windows platform. Database - SQL server 2014 User enters sales order using p4210 app, They enter all sales details and when they click on place order button, orders should be saved in system, Order nos are generated by system like next number. Bt after clicking on place order button, when they go to grid and search that order its not visible. This issue user facing not all the time, sometime it happened.
