Windows 2012 AD fails after power failure

Oracle VM Manager

Version: 3.4.6.2105

Build: 20181118_2105

OVS: 3.4.6

ZFS Appliance ZS3-2, model Sun Netra X4270 M3

Windows PV driver version 3.4.4

Problem/Issue:

We are loosing Windows Server 2012 Active Directory servers after a power failure. That is they either fail to boot, or they boot but files and disks gets corrupted. These are not recoverable, and we end up having to create new domain controllers from scratch.

We have encountered many blue screen errors (BSODs) in the long distant past, starting with OVM 3.2.9. We upgraded to 3.4.2, with the latest PV drivers and all the BSODs went away. Then the OVM Manager 3.4.2 constantly ran into database corruption problems due to "spawning of errors" issues, forcing us to upgrade to 3.4.6 (as per Oracle SR recommendation). But now we are encountering these Windows AD failures. This time around the errors are not BSODs though, and the VM failures are limited to domain controllers only.
