Running adgrants.sql is causing High Concurrency wait events

This occurs to our production env only.  When we run adgrants.sql as a pre-requisite of a patch, we could see the concurrency wait events starting to spike up and if we don't kill the adgrants session, database might hang or sometimes we will get calls from users that they got kicked out from EBS application.  Need help to fix this issue.

We are running on RDBMS version 11.2.0.4 with EBS R12.2.5.

Our adgrants version is:
