Unable to create new impairment

Hi,

In EBS Release 12.2.8 instance, when I am clicking 'New Impairment' button (Assets > Impairment > Create and Post), a blank web page opens with tab name 'Oracle Web ADI: Create Document'. I cannot see the page where I can select Excel version and create document successfully.

 

Please assist.
