BI Publisher Desktop - I can close Form/Insert window

I am wondering if anyone else has experienced this issue with BI Publisher Desktop - Word Add-On

When I select the Table/Form from the Insert toolbar it comes up as a separate window but there is no Close or Insert button(see below).  The window can't be resized either.

We have installed BI Publisher Desktop 12.2.1.0.0 Build 12.214.00.000 and are using Word 2013

Tools Release 9.2.2.4
