Conf' Call Gets dropped

Have an issue where an external caller (Phone A) calls in to Phone B,   Phone B then Conferences Phone C, at which point the external caller gets dropped.

Looking at the call traces, the issue occurs at the point of initiating the conference call.
An update gets sent out to the external caller, but that update is 'ignored' . It gets sent a second time, when the external party responds with 408.

The update that is being sent looks like this: (I have anonymised as much as possible).
Any body know why it would be ignored ?
