Is there a way to add a check box in the dynamic data sets?

I tried adding a check box on the dynamic data sets but the field cant be seen when searched. Is there a way to disable the field when the user selects a certain choice just like in the dynamic data sets? Please see below the pictures. I hope someone can help me.
