Duplicate PV is being used at boot - kernel panic / crash ODA x8-s

After running one of our ODAs for some time during a build we have gone through a power cycle and the ODA hits an issue with it's storage just after the login prompt is displayed.

A kernel dump is displayed and the server carried out a reboot - repeat.

Booting in single user mode allows us to gather more information that looks like an issue identifying boot disks / maybe multi-pathing?

Comparing with a working X7 I can see that the disk with UUID CoHLX5 ----- should exist twice with the same UID.

No obvious changes have been made at a storage level beyond adding one ACFS volume
