The file cannot be found on any specified file system

We have an odd behavior on one of our test environments (single server).

I can attach a file to any object (e.g. Contacts, or Accounts, etc).  It looks attached to me.  But when I try to open the file, I get this error message:

"The file xxxxxxxxx could not be found on any specified file system (SBL-UIF-00230)"

What are the areas that I have to look for?
