How to make input boxes on the web page reappear after it's not showing

On the Employee self service responsibility of Performance Management menu. It direct the user to the mid year review. whenever the user wants to add more information he clicks on the Mid year review add info. button then it sends the user to a form with 5 sections; 1. competence 2. challenge 3. learning and development 4. New objectives 5. Comments. All the sections should have input boxes to enable data entry. But the Learning and development section (web pages) Input boxes are not displaying. Hence users cannot enter their information on their learning and development needs.
