OMS and OMR Migration to New Server with Upgrade

Hi Team,

We Need to Migrate a OMS 12c Version  and its OMR 11.2.0.4 Version to a New Server which will be a new built and Version for same will be OMS 13.3 and OMR will be 12.2.

Both the Server are OL7. However we want the Data of OMR to be intact on new Server. Please let me know the Process and Steps of how to achieve the same.
