Oracle RAC DB - Need to replace all DB servers and upgrade RAC db

Oracle RAC database: 12.2 version

# of RAC nodes:                  4

OS:                                Oracle Linux 7

Storage:                        SAN

Size:                               82 TB - production    

I need to replace all 4 db servers(OL 7) and upgrade the 4 nodes RAC database. This is production 24x7 database.

Question: what is the best way to replace all 4 DB servers and upgrade the database from 12.2 to 19.2 with minimal down time? High level steps would be helpful.
