1099 - Company phone number

Company phone is not populating on 1099 form - it is in F0111 (who's who) attached to line ID # 1 and it is in F0115 Phone numbers as well also attached to line ID 1.  Company setup F0010 also has the company AN8 populated.  

All we get is Company ( ) on the R04512FA and on the media output

All setup was complete prior to running the AP 1099 work file build
