New integrator will only upload first row

I am creating/testing my first integrator.  It is a simple integrator that is taking values from spreadsheet and loading them into a custom staging table.  

When I preview/test, I enter multiple rows.   I get the following error, always on the last row -- and no rows are uploaded:  Cannot execute SQL Statement.   I click on the message and it says: A low level API call failed.  

So I enter 3 rows and the message always shows on line 3.  I remove row 3, try and upload and the same message shows on line 2.  I remove line 2, and the row will upload.
