Scheduled concurrent requests stops suddenly

All, Greetings for the day !

We are supporting legacy ebs application ( 11.5.8 ) , we observed that our scheduled requests ( 3 in numbers.. and at different dates to be specific ) were not scheduled for next run and had to manually schedule it again in Production.

Please note : When we checked the last concurrent program completed successfully and in the logfile and we do not see the next request id mentioned. viz next request is not scheduled.

Also, Audit is not enabled on the database to track what happened during that time.

It seems it is genuine bug but I'm not sure though.

Any pointers on how to proceed with the debugging ?
