Duplicate Default PMs created for the same asset activity combination

Hi  All,

I have an issue for one of my customer and its as given below.

Duplicate PMs are getting created in production for the same asset activity combination and the duplicates that are getting created are default=Yes.

The steps that the user follows is as given below.

1. Create a new activity using Activity workbench

2. Associate it to an asset

3. Launch Activity Workbench and open PM form

4. Create new PM, provide a name eg: CALCINER-PJ050-FBK001-12. for the asset CALCINER-PJ050-FBK001 and activity N05SYST-AD-P3M3_2020

5. Save the record, error encountered "PM name must be unique"

6. User then gives CALCINER-PJ050-FBK001-13, same error so this continues till the PM name is unique
