ACA Box 15 Shows Wrong Amount

Hi Everyone! I'm testing the ACA reporting process in our testing environment and have discovered that Box 15 is reporting an incorrect dollar amount for many employees-the value changes during the year on some, while reporting that one amount the entire year on others.  The value is 1 penny off the premium for one of medical plans from 2018, and doesn't match any medical premium amounts for 2019.  It's posted this incorrect amount regardless of coverage code or medical plan code.

Any ideas on what else I can look at to try and trouble shoot this? I appreciate your assistance!
