DB performance / Avarage active sessions

Dears,

I have got a AWR report from one of my clients complaining of slow Database performance, in the AWR i noticed that the elapsed time is 180 minutes while db tie is 21.78

the system has 32 cpus so its not CPU bound and avarage active session is 0.12 (DB time/ Elasped time) , meaning database is healthy

is my analysis missing somthing
