Part Time Hire that moves to Full Time - form not generating

I know that the SQL has to be customized to account for our codes - and we have PTF in the "eligible" list.  For some reason if they are hired as part time during 2019 and then move to full time, a form is not creating.  If they are full time and then move to part time and back to full time, it doesn't create the form correctly - but at least there is one.

Am I missing a place we need to account for PTF being eligible?
