Launching Form from Favourites Icon - Two forms/frmservlet Windows Appear

When I attempt to launch an EBS form from a favourites Icon I get two forms/frmservlet windows launched followed by two 'Oracle Developer Forms Runtime -Web' windows which contain the error message 'FRM-92101 There was a failure in the Forms Server during startup.'

However, when I launch from the drop down menu the core form launches without any issues.
