Mass Additions Create Report completed in ERROR.

Dear Team,

We ran Mass Additions Create Report and it completed with ERROR "APP-FND-01624: The token value NULL for CODE is inconsistent in the user exit: FORMAT_CURRENCY."

I checked in metalink and referred the document  id 763082.1. But in our system I can see the file fa115cp.ldt version as 120.15 which is the latest one in R12.1.3.

Can anyone please help me on this to resolve.
