I want migrate oracle database 12 c from one Linux server to another Linux server. source server is

I want migrate oracle database  12 c from one Linux server to another Linux server. source server is having standalone asm environment with 5 diskgroup and target server is also having one new disk group but I want to move source asm diskgroup into target asm with same diskgroup . oracle GI and database software is standalone  with same version that is 12c but source os is  oel 6 and target  os is oel 7. Please provide me steps for this activity.
