Supplier Registration Embedded Form W-9

Looking for any advice on embedding a fillable Form W-9 in the Supplier Registration Identifying Information page.    Request is to have a link to a Form W-9 that the Supplier could complete and select a save or upload button to save directly to the Supplier Registration attachment page.  The goal is to keep the Supplier from having to save a document to their computer and then upload as an attachment.
