Forms Server - Need exchange rate type LOV for Form APXPAWKB for Personalization

Hi,

We have tried numerous times to open up a particular form (APXPAWKB) so that we could Personalize the LOV (Record Group for Query) for the Exchange rate type.

Could someone please send me the sql for the query due to us having issues with Forms developer?
