Linux server restarted automatically -how to know the reason

Good Morning ,

We are using R12.1.3  on Oracle Linux OS

Rcently  last week we had problem of   application server  system - is restarted automatically.. Then DBA need to start all the services.

Now yesterday and today again the  application server system is restarted and we getting error -unable to connect when loading the URL

root password  - we are using the same password for the last  few years - suddenly now getting message access denied

How to find the  reason for server  restarted and root password  changed ?
