Is there some additional configuration to get the actual attachment file from a name change to show

We are 8.56.15, PT 9.2.30 and are just starting to test ESS and name changes including attachments.   I have the security I think set correctly and the configuration so that the attachment saves for the employee and I can see there is a file on the approval.    When I as the approver go to view that attachment there is nothing there on the panel.
