Hi Mario,

I understand this can happen over a period of time especially when multiple migrations and upgrades occurs.

Looks like the problem lies here and my earlier suspicion/comment is true. This may not always translate to individual fail group. Note the ASM redundancy is solely determined by fail group.

The picture is helpful. Please confirm the dependency or relationship of 'D1', 'D2', 'D3' with "F61" and so on. This will help in next steps to fix your issues. It looks to me that these are LUNs coming from SAN1 and similarly from SAN2.
