Request information regarding Certificate for Spousal Benefit Surcharge

HCM 9.2 Ben Admin.  How do you set up Certificate for Spousal Surcharge for benefits i.e. spouse is employed and has benefits offered through his / her employer but waives those benefits and chooses the spouse's employer benefits.
