Unable to create PM schedule using create_pm_def

I am trying to use the eam_pmdef_pub.create_pm_def API to create a Prev. Maint Schedule.  This is for data conversion.  After import I need to be able to open the new record in the 'Enterprise Asset Management --> Preventative Maintenence --> Schedule Definition" form and find my schedule that I created.  I found a script on metalink (2025230.1)) but I am not having any luck..   After I run the script I see the record in the EAM_PM_SCHEDULINGS table as I would expect but I am not sure what I am missing to make it not show up in the form.  Once I get it showing up in the form I am sure I will have more questions about what all the values mean, but I can't move forward with anything without it showing up in the form.  This is the script:
