ORA-20621 while adding DB instance target with guided process

Hi,

My attempt to manually add new RAC instance monitoring target with guided process in EM13.4 fails in its final stage when hitting the save button with error message:

Target Saving - Failed.

Failed

'SID' :  ORA-20621: Cluster member with guid 2D8EAB3349CE382BC762DC087F26561C already belongs to clusters:  054E14F63C7C51649C07207071

How can I solve this issue?
