Classic Navigation bar entries not displayed correctly

After migrating an Apex 4 application to version 19.1 The entries on the Classic Navigation Bar are for a few pages not being displayed correctly.

The application has a number of entries in the classic navigation bar - usual things such as [logout] [User Guide] etc. The position of the navigation bar is set to Top. On most pages this is displayed correctly. However of a small proportion of pages the top half of the entries is hidden. So it appears as if if it is being hidden behind the IE Command Bar at the top of the page. I have tried other browsers (Chrome, Firefox) but the effect is the same.
