Documentation for writing a table conversion?

Hello -

We are adding columns to an existing table, but need to retain all the of the existing data in the table when the new specs are applied.  Is there documentation on how to write a table conversion that will apply the new table specs, generate the table and copy the old data back in?

I haven't written a TC in over 15 years and cannot remember how to do this.
