I am upgradig grid and database from 11.2.0.3 to 12.1.0.2 version , I have upgraded grid but facing

I am upgrading grid and database from 11.2.0.3 to 12.1.0.2

Earlier it was two nodes but we have converted this database into single node at last dec2019 ...

Now i am upgrading this database, Gird upgrade has completed successfully but while upgrading database getting below error.
