Sales order missing

Hi Folks,

We are on JDE 9.0 with TR 9.1.5.5, all the servers are on windows server 2012 R2 64bit with SQL database 2014 - 12.0.4100.1 (X64).

We are facing strange issue where users are punching sales orders one after another and one of the order gets missed.

After clicking place order button in P4210, the process gets finished but when they go back and check then the same order is not showing.

Also, most of the time users creates few sales order one after the other. Suppose a user creates  three orders in line, in that second orders didn't reflects in the system and strange part is they don't see any timeout or lag. Once they click the place order button they are through then they will go for next order.
