Inventory report that includes requestor name

I am having difficulty finding a seeded inventory report that includes something along the lines of

Account - 5632

Project # 0000

Item # 123456

Description - Shoe

QTY - 1

Date - 25-Sep-2012

Unit Price - $0.00

Total Amount - $0.00

Requested BY - Bob

I see that the Material Account Distribution Detail report has something similar, but it is not a clean excel report, and does not include the transaction number or requestor name.

Any suggestions?
