Random logon failures logged to UNIFIED_AUDIT_TRAIL

Monitoring our UNIFIED_AUDIT_TRAIL we occasionally have an entry with ORA_LOGON_FAILURES audit policy and a RETURN CODE = 604 with no other identifying information as to what caused the logon issue. It randomly happens across different PROD databases. It pops up inconsistently and randomly across all databases. It can be successful for weeks on end and then it will randomly occur 1 day and then not fail again. I have checked Alert Log around time of the logon failure and I see nothing. Should there be additional errors in the error stack to identify the problem? I understand that 604 error
