Since upgrading to 12.2.9 some PM's will not generate and some will. Any thoughts or insight?

Since upgrading some PM's will no longer generate unless you go in and close the previous work order.  This has never been an issue before.  Also, this happens randomly to PM's.  It does not happen to all of them.
